Insurance Solutions Agency is authorized to collect personally identifiable information (PII) from you authorized by CMS. All PII information we collect will be used for enrollment purposes.
If you choose to give us your PII, we may share this information with the insurance provider of your choice for the completion of the enrollment process.
The choice to share your PII information is voluntary. If you choose not to share your PII information or to respond to only certain questions, the discretion of accepting/denying the application will be left up to the discretion of the insurance provider of your choice.
